Your employer or the government agency that issues your payroll checks is responsible for establishing direct deposit on your behalf. You will typically fill out a basic form requesting the following information, and they’ll do the rest.
You will need to provide the following information on our Payroll Authorization Form:
- Bank routing number: 284383886
- Your account number
- A voided check or deposit ticket to verify the routing and account numbers
- The addresses of your financial institution
